Must be a Utah Resident
Must have at least one qualifying condition
Apply and register on the EVS portal
Meet with a qualified medical provider (QMP)
Eligibility has been certified by your QMP
Pay a 15$ fee to the state for your card
Note: If a patient is a minor under the age of 21 or an adult over 21 but does not have a qualifying condition, the application must be reviewed by the Compassionate Use Board. A minor cannot receive a medical cannabis card unless they have a qualifying condition and their parent or legal guardian qualifies for a medical cannabis guardian card.
Currently, insurance companies will not cover evaluations directly or indirectly associated with cannabis recommendations due to cannabis still being a federally illegal substance.
You will need to register with any Utah Medical Cannabis Pharmacy you plan to visit. This can be done ahead of time with some dispensaries, or you can register in person as a walk-in.
On your first visit to a Medical Cannabis Pharmacy, you will need to go in person or have a virtual visit and consult with a pharmacist. After that, you are welcome to use the drive-thru and delivery options (when applicable).
Log into your portal on the EVS site.
Click "tracking Inbox," then your name below in the box that pops up.
Scroll down to the bottom of the page and click the blue "Save & Submit for Certification" button. Your registration status should now say "Awaiting Certification."
Call or email to set up a renewal appointment
Pay $15 fee to the state once your QMP submits your application and wait for your card to be emailed to you via the State of Utah.
